The 2009 Primary Care Mini Grant deadline was February 2, 2009. Successful candidates will be notified by Friday, February 13, 2009 by email.

 
Purpose: To improve primary health care access and reduce health disparities by identifying and assisting Arizona medically underserved communities with community development activities such as capacity building and strategic plan development.
  
Background: In October 2000, the Bureau of Health Systems Development (HSD), also known as the Arizona Primary Care Office (PCO), and the Arizona Association of Community Health Centers (AACHC), also known as the Arizona Primary Care Association (PCA), partnered together through the Federal Cooperative Agreement to provide community development activities for Arizona Medically Underserved Areas (AzMUAs) to improve primary health care access and reduce health disparities.
  
Goals: To strengthen and improve primary health care access for Arizona medically underserved communities.
  
Activities at local level:
  • Identify communities who exhibit a community readiness for development activities
      
  • Work with communities in early stages of community change
      
  • Assist communities in leveraging dollars
      
  • Develop and improve community capacity
      
  • Facilitate community discussions
      
  • Strengthen the safety net
      
  • Increase types of services available in communities
  
Activities at state level:
  • Build networks
      
  • Build partnerships
      
  • Expand service delivery
