Reporting Fraud and Abuse

If you suspect a Tribal/Regional Behavioral Health Authority (T/RBHA), a T/RBHA provider or recipient within Arizona's public behavioral health system of committing fraud or abuse, please report it by contacting the Arizona Department of Health Services/Division of Behavioral Health Services' Fraud and Abuse Hotline at (602) 364-3758 or 1-866-569-4927.

You may also mail a report of fraud or abuse using the:
